I understand that the $240 football camp registration fee is non-refundable.

Model Release

I agree that my child may be photographed or videotaped during activities with Elevation Athletics, and I grant permission for these images to be used for online promotional and advertising purposes.

Waiver and permission to participate in physical activities and authorization for emergency medical care

I, parent/legal guardian of the above-named child, hereby give my permission for him/her to participate in all athletic activities provided by Elevation Athletics. I am aware that it could result in some contact and may require strenuous physical activity. I assume all risk of hazards incidental to such activities. I hereby waive, release, absolve, indemnify, and agree to hold harmless Elevation Athletics, LLC, and its respective organizers and coaches from any and all claims arising out of his/her participation in or being transported to or from the same, whether the result of negligence, or any other cause, except to the extent and amount that may be covered by accident or liability insurance. In the event of injury or illness to my child, I hereby grant authorization for emergency medical care prescribed by a licensed EMT on the premises. This emergency care may be given under any conditions which are necessary to preserve the life or well-being of my child/dependent.
